Week Six

Introduction

As we already know safety rules and regulations are very important for both employers and employees. This week we will talk about accidents and incidents. You will listen to an accident/incident investigation and write an accident/incident report.

Outcomes:
After this unit you should be able to:

• understand an accident/incident investigation,
• participate in an accident/incident investigation,
• understand an accident/incident report,
• write an accident/incident report.

Tasks and Dates

This unit consists of four activities. In the first activity you will share your experience with accidents and/or incidents with your colleagues. Then you will listen to an accident/incident investigation interview and complete the listening comprehension questions. After that you will try to make your own questions for an accident/incident investigation interview. Finally, you will write an accident/incident report.

Activity One

Go to FORUM 6.1 and answer the following questions:

Have you ever had an accident? What happened?

Then write a definition for:

• accident
• incident

To check if you understand the two words go to QUIZ 6.2 . Compare the short texts with your definitions.

Activity Two

This is a listening activity.
1) Listen to a recording and make notes.Explain what happened.

2) Listen to the recording again and complete the missing information in the employee accident report form. Check your answers by completing QUIZ 6.3.

Activity Three
