Accident Report

Name of injured person: John Welsh
Job title of injured person: maintenance worker
Date and time of the accident: 22^nd June, 2021
Description of injuries: severe burns to left arm and leg and the left side of torso, electric shock, ventricular fibrillation


Hospital treatment: Y/N Cardiopulmonary resuscitation (CPR) performed, several weeks in ICU (Intensive Care Unit)
How the accident happened: rode 12 ft above the floor on a motorized lift, work on a 277-volt light fixture, began to strip the hot wire using a wire stripper, electricity passed from the live wire to the stripper and then into his hand and body


Causes of the accident: power supply to the lights not turned off, mistake in installation, hot and dead wires confused, a line fuse removed
